Helping you coordinate

Your Supports

Support Coordination is an NDIS support that aims to build the capability of an NDIS participant to effectively use their NDIS plan to achieve their goals. A support coordinator is a person who delivers support coordination which comes under the category of capacity-building support.

This Support service involves assisting participants in coordinating their NDIS supports. At Centre Care, we understand our participant’s needs are different from each other. Our support coordinators will help organise your NDIS support so that you can get the most out of your plan. We’ll make sure your needs are met and we help you achieve your goals. Some of the things our support coordinators can assist you with include;

  • Report to the NDIA
  • Budgeting the NDIS Plan
  • Connecting you to local support services
  • Attend Appointments
  • Capacity Building Skills
